Information for Parents

• Inform the school nurse of any medical condition that might affect your student during a school day (Asthma, ADHD/ADD, Life Threatening Allergies, Physical Disabilities, Surgery, etc.) This information needs to be updated every school year. • Notify the nurse if there are medication changes for an ongoing medical condition. • A parent's note for P.E./Activity restrictions is good for 3 days. A Dr.'s note is needed if it will be longer than 3 days. • A change of clothing should be kept in every Pre-K and Kindergarten student's backpack. • Make sure that the school nurse ALWAYS has a way to reach you, in case on emergency or illness occurs. Current phone numbers are very important!! ** Make sure your child eats a good breakfast every morning.
